Plasma "cortisol" response to Synacthen in patients on long-term small-dose prednisone therapy.

چکیده
Material Patients.-39 patients were studied, all but one of whom had taken prednisone 5 mg./day for periods of from 8 weeks to 8 years. One patient (Case 31) had her dose increased from 5 to 7 mg./day 8 months before the test. With the exception of two who had polymyalgia rheumatica, all had rheumatoid arthritis. Of these 39 patients, nineteen were selected at random and studied again after a year to discover the effects of continued or altered prednisone dosage upon adrenal function. Control.-Twenty normal hospitalized controls of comparable age range were studied under the same test conditions as the patients.

Recovery of adrenal function in a patient with confirmed Addison's disease
UNLABELLED Addison's disease is a condition characterised by immune-mediated destruction of the adrenal glands leading to a requirement of lifelong replacement therapy with mineralocorticoid and glucocorticoid. We present a case of a 53-year-old man who presented at the age of 37 years with nausea, fatigue and dizziness. He was found to have postural hypotension and buccal pigmentation. His pre...
متن کاملNormal cortisol response on low-dose synacthen (1 microg) test in children with Prader Willi syndrome.
INTRODUCTION It has been postulated that central adrenal insufficiency (CAI), resulting from hypothalamic dysfunction, may contribute to the increased unexplained death rates in Prader Willi syndrome (PWS). A study using the overnight metyrapone test reported a 60% prevalence of CAI in children with PWS. We used a low-dose Synacthen test to screen for CAI in children with PWS. METHODS We stud...
متن کاملAntiprogestins, a new form of endocrine therapy for human breast cancer.
The antitumor, endocrine, hematological, biochemical, and side effects of chronic second-line treatment with the antiprogestin mifepristone (RU) 486) were investigated in 11 postmenopausal patients with metastatic breast cancer. We observed one objective response, 6 instances of short-term stable disease, and 4 instances of progressive disease. Mean plasma concentrations of adrenocorticotropic ...
متن کاملComparison of two high dose corticosteroid aerosol treatments, beclomethasone dipropionate (1500,Ig/day) and budesonide (1600 ag/day), for chronic asthma
Twenty eight patients with chronic asthma took part in a double blind single crossover controlled trial of inhaled budesonide and inhaled beclomethasone dipropionate, using high doses of 1600 pg and 1500 ug daily respectively. Both drugs were administered by pressurised aerosol inhaler; the inhaler containing budesonide and its matching placebo were fitted with a collapsible spacer device. Ther...
متن کاملTests of adrenal insuYciency
Aim—In suspected adrenal insuYciency, the ideal test for assessing the hypothalamo–pituitary–adrenal axis is controversial. Therefore, three tests were compared in patients presenting with symptoms suggestive of adrenal insuYciency. Method—Responses to the standard short Synacthen test (SSST), the low dose Synacthen test (LDST), and the 08:00 hour serum cortisol concentration were measured in 3...
